Can an audit be updated (exception registered) after the auditor has signed off? Thanks in advance.

If you mean can you change the answer to an audit question, to my knowledge you cannot. As an assistant area auditor, if the auditor incorrectly answered a question, I just mark it as incorrectly answered (that's a radio button option) and then add a comment explaining why the answer should be different.
