One of our ward clerks has tried to get baptismal information for a child who was baptized in another state, but the parents do not want to share the information. The clerk thought he could get the information, so he checked the child's name during the membership audit and now there is an "unrecorded baptism or confirmation" exception. The clerk is going to do another audit and not check the child's name so the exception will clear out, but is there a way to make a note so current and future leaders know the situation?

No, I don't think there's any way to put the note on whatever record exists.
I would point out Handbook 38.2.6.1. The baptism ordinance isn't valid if it wasn't recorded. I would have the bishop communicate that with the parents.
If it has been recorded and you just need the information to move the record in, I'd call support. They can probably locate the record.
